LIFTT Brings You Options Counseling

LIFTT will be kicking off a new program called Options Counseling on June 5th, 2017. Options Counseling is available to individuals with disabilities age 18 and older. In the event that the individual is not able to participate, their representative or caregiver who requests services and support can participate on their behalf.

Options Counselors assist you  in exploring all available community-based options before institutional placements. They help individuals weigh the pros and cons of the services available, including accessing public benefits for which they may qualify. They will assist with the decision making, including development of an action plan that supports and works for the individual. Services are available to help in a variety of living situations, whether it be in-home or facility.

Individuals meet one-on-one with a counselor to discuss what their short and long term care needs are, help them prioritize their needs, develop action plans and connect the individual to available options and services. Individuals of all incomes and assets are qualified to receive services regardless of their ability to pay. If you or someone you know is asking these questions :
